Becoming a Client
CARD serves individuals of any age with: Autism Spectrum Disorders (ASD) including Autistic Disorder, Pervasive Developmental Disorder Not Otherwise Specified (PDD-NOS), Rett’s Disorder, Asperger’s Disorder, and Childhood Disintegrative Disorder Related Disabiliites including deaf-blind (hearing and vision impaired), and hearing impaired or vision impaired with other disabling conditions CARD will accept referrals from medical and educational professionals, family members, and individuals in need of our services. To become a client a referral packet must be completed and submitted to FSU CARD. You can request a referral packet by calling (800) 769-7926 or by downloading the appropriate forms below.
